Enrollment
Children should be
registered for the class that best matches his
or her age and ability. Enrollment is on a
first-come basis and ends on November 19, 2011
for students expecting to participate in the
annual recital. After November 19, 2011,
students can only register for open enrollment
classes. All students must complete an
enrollment form.

Registration Fee
A
one-time fee of $40.00 per family and first
month class fees are due at registration.
Registration fee is on-refundable and
non-transferable. Registration Discount:
Anyone registering before or during the month of
September will receive 50% off their
registration fee. No other discounts may
be used with this discount.

Class
Withdrawal Form
To withdraw from a
class at any time during the program, please
turn in a completed Class Withdrawal Form to the
Front Desk. If a Class Withdrawal Form is
not completed, you will be responsible for
paying tuition through the end of the program.

Class
Attire
Appropriate dancewear
and shoes are mandatory for any class.
Students should come to class dressed and ready
to participate.
Girls:
Leotards, tights, skirts or yoga pants/legging,
fitted T-shirt or tank top;
baggy T-shirts
and sweats are not permitted.
Boys: Sweat
pants, T-shirt, movable clothing; baggy
tops or pants, basketball shorts or jeans are
not permitted.
Adults: Clothes
you can move in easily.
Absolutely no street
shoes allowed on dance floors

Costumes
Our costumes are age
appropriate! Costumes are ready made and
involve little or no sewing by the parent.
First payment is due October 16, 2011 and final
payment is due November 5, 2011. Full
payment must be received before costume will be
ordered, and accounts must be current in order
to receive costumes.

General
Studio Policies
Severe Weather Days:
Classes will be cancelled if weather is too severe.
A pre-recorded message will be on the studio voice mail,
and if time permits, class cancellations will be
distributed via e-mail or WCCO.
Missed Classes: No
credit given for missed classes.
Past Due Balances: When
an account is past due $100 and individuals and $200 for
families, students will not be allowed to participate in
classes until account is brought current. All
accounts must be current in order for student(s) to
participate in annual recital.

Dance Tuition is a
Tax Credit
Under Minnesota State K-12
Education Subtraction and Credits: you are able to
subtract/credit for your dance tuition.
Keep your receipts; we do not
give year end statements.
Donations are Tax
Deductible
Hollywood Studio of Dance is a
501(c)3 non-profit organization and is qualified to
receive donations.
If you give to the United Way
foundation through your employer, you can designate a
contribution to us.
Your donation helps us help
families in need.
